Most quality dashboards I've inherited over my career share a common problem: they're excellent at telling you what went wrong last month and nearly useless at telling you what's about to go wrong next week. They measure outcomes — defect rates, scrap costs, customer returns — and present them as if measuring the problem is the same as managing it.

After years of building scorecards, running Kaizen events, and sitting in a lot of QBR meetings, I've narrowed my essential tracking list down to five metrics. Not because the others don't matter, but because these five have the highest signal-to-noise ratio for someone trying to prevent quality failures, not just count them.

1. First Pass Yield (FPY)

First Pass Yield is the percentage of units that complete the entire production process without any rework, repair, or rejection. It's my single most important quality metric because it captures hidden factory cost that defect rates alone miss.

A plant can have a very low final defect rate — say 0.3% — and a terrible First Pass Yield of 72%. Those numbers tell completely different stories. The FPY tells you that nearly 30% of your product is being touched more than once, consuming labor, materials, and floor space that the customer isn't paying for.

Target: Track by product line and by process step. A step with FPY below 95% deserves a root cause investigation.

2. Escaped Defects (Customer Rejection Rate)

Escaped defects — defects that your inspection process failed to catch before they reached the customer — are your most expensive quality failures. They carry warranty costs, expediting costs, potential line stoppages at your customer, and relationship damage that doesn't show up on any financial report.

More importantly, escaped defects tell you something specific: your detection system failed. Every escaped defect should trigger a review of the relevant FMEA and control plan to ask why the current controls didn't catch it. That review is more valuable than the defect count itself.

Target: Zero escaped defects is the goal. Even one per quarter in a mature quality system deserves a formal 8D.

3. Corrective Action Cycle Time

This one rarely appears on quality dashboards, and it should. Corrective Action Cycle Time measures the average elapsed time from problem identification to verified closure of a corrective action. It's a direct measure of your organization's problem-solving velocity.

A shop floor with a 90-day average corrective action cycle time isn't managing quality — it's documenting it. Problems that take three months to close have almost certainly recurred multiple times during that window. Driving this number down to 30 days or less is one of the highest-leverage improvements a quality team can make.

Target: 30 days or fewer for standard issues; 45–60 days for complex 8D investigations with supplier involvement.

Why This Metric Changes Behavior

When corrective action cycle time is visible on the quality dashboard, it creates accountability without finger-pointing. The team sees the number and asks "what's stuck, and how do we unstick it?" That's a much more productive conversation than reviewing a defect count.

4. Cost of Poor Quality (COPQ)

Cost of Poor Quality translates your quality performance into the language that gets leadership attention: dollars. COPQ captures internal failure costs (scrap, rework, reinspection), external failure costs (warranty, returns, field service), and appraisal costs (excess inspection driven by process instability).

Most organizations significantly undercount their COPQ because they only capture the obvious costs — scrap and warranty claims. The hidden costs of rework labor, expediting, and customer-relationship repair are rarely fully accounted for. When I've built comprehensive COPQ models, the real number is typically 2–4 times the initial estimate.

Target: COPQ as a percentage of sales. World-class manufacturers typically operate below 1–2%. If you don't know your current number, calculating it for the first time is usually a sobering and motivating exercise.

5. On-Time Closure Rate for Gemba-Identified Issues

This is the metric I care most about as a CI leader, because it measures cultural health rather than just process performance. It tracks what percentage of issues identified during gemba walks, daily huddles, or operator observations are closed within a defined window — typically 72 hours for simple issues, 30 days for complex ones.

When this number is high, it means your organization has the habit of finding and fixing problems close to where and when they occur. When it's low, it means problems are being identified and then stalling — which quickly trains people to stop raising them.

Target: 85%+ on-time closure rate. Below 70% is a culture signal, not just a process signal.

What to Do With These Five Numbers

Track all five in a simple one-page dashboard reviewed weekly by the quality and operations leadership team. Trend them over 13 weeks so you can see direction, not just current state. When any metric moves in the wrong direction for two consecutive weeks, that's your trigger for a focused problem-solving session — not a wait-and-see posture.

The goal isn't a perfect score on any individual metric. The goal is an organization that sees problems clearly, responds to them quickly, and gets systematically better every month. These five metrics, tracked honestly and acted on consistently, will tell you whether that's happening.
